What is a Life Plan (CCRC) Community?

A Life Plan community, formerly known as a Continuing Care Retirement Community (CCRC), provides residents with independent living as well as a continuum of care on premises.

Life Plan communities offer compassionate care at predictable monthly fees rather than providing those services at fluctuating market rates. The result? Residents can live for today while planning for tomorrow. 

Generally, Life Plan communities offer 3 to 4 different types of residency and/or care: Independent Living, Assisted Living, Memory Care and Skilled Nursing. From independent living to assisted living and memory care to rehabilitation services, Life Plan communities appeal to retirees seeking long-term care with the expectation that their healthcare needs may increase with age. This is also an attractive option for couples who require different types of on-site care as their needs change.

Independent Living

Independent living means just that – independence. Residents can choose from a number of social activities and outings, educational programs, cultural events, fitness options and concierge healthcare, which cater to active retirees and thereby ensure longer, healthier, and happier lives. All while living in a residence of their choosing .

Assisted Living 

If the need arises, Assisted Living provides aid with the activities of daily living. Caregivers provide care such as help with bathing, dressing and grooming.  It’s considered an intermediate type of care and is provided to people who may need extra help with meal preparation, medication management, and other tasks. Residents maintain their independence by living in comfortable apartments while receiving assistance from experienced, compassionate caregivers including on-staff RNs and CNAs. 

In some Life Plan communities, Assisted Living residences and services may be enhanced through an Extended Congregate Care (ECC) license, which provides nursing services above those of a standard license.

Memory Care

Memory Care services in a Life Plan community provide specialized, compassionate care to residents with varying degrees of dementia or Alzheimer’s and their families. When a loved one experiences memory loss, person-centered care utilizing unique tools designed to engage residents in purposeful communication and camaraderie with their neighbors which, in turn, helps them feel safe, secure and valued.  

Skilled Nursing

Rehabilitation or Transitional Care 
Skilled nursing services are provided on a short-term basis for people leaving the hospital following surgery, illness or accidents. Offering a variety of physical, speech and occupational therapies, it is designed to help patients return to where to their normal lives and their original place of residence. 

Long-Term Care 

Long-term care is provided to those needing 24-hour nursing care on an ongoing basis. Services may also include a range of physical, speech and occupational therapies, as well as respite, palliative and hospice care.

At Moorings Park you’ll find all types of care, from independent living on all three campuses to Assisted Living and Memory Care. Our ECC licensure provides residents with nursing services beyond those of a standard Assisted Living license. Skilled Nursing is offered at the Chateau on Moorings Park’s original campus.
