Transitions

Moorings Park strives to provide its residents with the tools for successful aging. As part of this mission, our Transitions program is designed to alleviate the stress and distress of alterations in life status such as:

  • A move from home to Moorings Park
  • A move to Orchid Terrace or The Chateau
  • The death of a loved one
  • A change in health status
  • A change in mobility
  • A change in cognition
  • A change in financial situation

A task force of residents, community partners such as Avow and Vitas hospices and partners representing The Center for Healthy Living, Wellness, Spiritual Life, Marketing and Assisted Living comprises our Transitions program.

First Steps

First Steps

The members in this program welcome new residents by proactively engaging and providing helpful resources as the resident moves into Moorings Park, familiarizing the new residents with their new home and community, and answering any questions about accessing services, staff and Moorings Park programs.

Residents in Caring

Residents in Caring

Our Residents in Caring programs were designed to encourage Moorings Park residents to provide support and care for others especially those residents who are new to Moorings Park, newly single or who are going through a life transition. One resident per floor, per building, across the campus is selected to serve as a representative for The Residents in Caring Committee.

Bereavement Support Program

Bereavement Support Program

To offer Moorings Park residents the opportunity to address their emotional, spiritual, and transitional concerns in a safe, non-judgmental environment. The on-going bereavement support group meets twice a month.

Special Care for Residents

Special Care for Residents

Special Care includes a birthday card being sent, a care note sent at the first anniversary of the death of a spouse/significant other, support of those newly single through death or a transfer of spouse to higher level care, welcome home notes sent to residents as they arrive home after a hospitalization, visits while hospitalized, table topics.
