To qualify for the award, applicants must participate in a consumer satisfaction process involving residents, family members and guardians, as well as demonstrate the involvement of families and members of the community in the facility, and have a stable workforce. They also must have an outstanding record with the State Long Term Care Ombudsman Council within 30 months preceding their application.
